General & Preventive Healthcare

Preventive care is about more than treating illness—it’s about helping you stay healthy. Community Integrated Health provides routine health screenings, physical exams, preventive care, and ongoing guidance to help identify health concerns early and support your health at every stage of life.

Acute Illness & Injury Care

When you're sick or injured, you don't always need an emergency room—but you also shouldn't have to wait weeks to be seen. Community Integrated Health provides evaluation and treatment for many common illnesses and minor injuries, with same-day appointments available when possible.

Children & Adolescent Healthcare

Growing kids need healthcare that grows with them. Community Integrated Health provides care for children and adolescents, from routine checkups and physicals to common illnesses, minor injuries, and ongoing health concerns. Our goal is to provide families with accessible, personalized care while supporting healthy development through every stage of childhood and adolescence.

Routine childhood immunizations are not currently available at Community Integrated Health. Families may be referred to local community resources for recommended vaccinations.

Women’s & Men’s Health

Healthcare needs can change throughout every stage of adulthood. Community Integrated Health provides personalized care for common men’s and women’s health needs, including preventive screenings, sexual and reproductive health concerns, urinary symptoms, menstrual and menopausal concerns, and other routine health needs. Care is focused on open conversations, appropriate screening, and helping you make informed decisions about your health.

Fertility evaluation and treatment are not currently offered. Referral to specialty care may be recommended when appropriate.

Behavioral Health Support

Mental health is an important part of overall health and deserves the same attention as physical health. Community Integrated Health provides evaluation and ongoing management for common behavioral health concerns, including anxiety, depression, sleep concerns, and ADHD. Medication-assisted treatment (MAT) for substance use disorders may also be available when appropriate.

Patients with more complex psychiatric conditions, including bipolar disorder and schizophrenia, may require initial evaluation and treatment planning by a psychiatric mental health specialist. Once a patient is stable with an established treatment plan, Community Integrated Health may provide ongoing medication management and supportive care. Significant changes in symptoms or treatment needs may require re-evaluation by a psychiatric mental health specialist.
